Avoid the Serious Side Effects of Epidural Steroid Injections

Regenexx PL-DiscThe recent news about epidural steroid injections (ESIs) has not been good. Recently, a meta-analysis of 23 clinical studies published from 1970 to 2012 on ESI for sciatica was published, that questions just how effective ESIs really are for this condition. Sciatica is probably the most common reason that the procedure is performed and there is a wide spread belief by the medical community regarding its effectiveness. The new study found that when ESIs were compared with a placebo injection, ESIs provided a small short term improvement in pain and disability. There was no evidence of any improvement after one year.

These results align with a similar review of published medical literature by a committee formed by the American Academy of Neurology in 2007 that highlighted the lack of evidence of long term benefits of ESIs. The study also discovered significant controversies regarding the best technique for delivering these injections, the best medications and dosage to use, and the frequency of injections.  An estimated 9,000,000 EPIs are performed in the US each year, with mixed evidence of short term benefits and zero evidence of long term benefits. This has resulted in a growing number of physicians in the US losing support for ESIs.

Multiple studies have shown that for most people, abnormal findings on MRIs and other tests do not correlate well with the presence or severity of pack pain or sciatica. In fact, many patients who have actual disc herniations and undergo surgery, whose outcome would be expected to free nerves that are irritated or impinged, do not gain relief and suffer from the multitude of negative effects of the high doses of steroids. It seems that the use of ESIs is based largely on a theory for which there is little supporting research.

We are very enthusiastic about the Regenexx PL-Disc and Regenexx PL-DDD next generation regenerative therapies. These new technologies have been demonstrated to have improved reductions in pain and increase function when compared to ESI’s. This is the future for treating spine conditions that have failed more conservative therapies, demonstrating considerable improvement and lower risk than any other intervention available.
